29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At the jobsite, employees operating a Metabo nail gun to perform roofing activities without eye and face protection exposing him to the hazard of being struck-by flying objects.  Jesus Vega Acosta was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1697657, Citation Number 1, Item number 1, and was affirmed as a final order on October 16, 2023, with a final abatement date on October 6, 2023,  with respect to a workplace located at 1849 Rough Ridge Drive, Midlothian, TX 76065.  Jesus Vega Acosta was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1529549, Citation Number 1, Item number 1, and was affirmed as a final order on June 9, 2021, with a final abatement date on May 7, 2021, with respect to a workplace located at 2316 Elm Place, Northlake, TX 76247.

At the jobsite, employees installing roofing material to a new residential house were wearing fall protection but was not tied off, exposing employees  to an unprotected fall hazard at a height of over 6 feet to the ground below.
