29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 4, 2024, employees who process meat on the Kosher Cooler to Kosher Fabrication conveyor at the WR Reserve facility at 1009 West M St, Hastings, Nebraska were exposed to an in running nip point due to a 2 inch wide gap between the conveyor and the cutting table while they are pulling product from the line.

29 CFR 1910.303(b)(7)(iv): Electric equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: On or about April 4, 2024, employees who work in the fabrication area of the WR Reserve facility at 1009 West M St, Hastings, Nebraska were exposed to electrical shock hazards from three unprotected electrical junction boxes. The electrical boxes had missing covers which exposed wiring to a wet environment.
