29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) New residential construction located at The Oaks at Dawson Subdivision, 177 Water Oak Way, Lot #86, Dawsonville, GA 30534, Roof: On or about 07/09/2024, the employer exposed employees to fall hazards of approximately 18-feet, 10-inches to the lower ground level,  in that the ladder side rails did not extend at least 3-feet past the roof landing to allow for safe access on and off the roof.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New residential construction located at The Oaks at Dawson Subdivision, 177 Water Oak Way, Lot #86, Dawsonville, GA 30534, Roof: On or about 07/09/2024, the employer exposed employees to struck-by hazards while operating a pneumatic nail guns, in that protective eyewear was not provided to employees engaged in roofing activities while operating these tools at 120 PSI.  Freddy's Roofing and Home Repair, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1655676, citation number 1, item number 1, and was affirmed as a final order on 07/12/2023, with respect to a workplace located at The Adagio Community, Cadenza Circle, Lot #42, Dacula, GA 30019.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) New residential construction located at The Oaks at Dawson Subdivision, 177 Water Oak Way, Lot #86, Dawsonville, GA 30534, Roof: On or about 07/09/2024, the employer exposed employees to fall hazards of approximately 18-feet, 10-inches to the lower ground level, in that fall protection systems were not provided while the employees were engaged in roofing activities.   Freddy's Roofing and Home Repair, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1655676, citation number 1, item number 2, and was affirmed as a final order on 07/12/2023, with respect to a workplace located at The Adagio Community, Cadenza Circle, Lot #42, Dacula, GA 30019.
