29 CFR  1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards.  a) On or about 08/15/2024, the employer did not ensure that employees storing acetylene cylinders in an entryway in the cotton warehouse had secured them via chain or other methods as to prevent the cylinders from being knocked over or damaged by passing powered industrial vehicles and/or employees.

29 CFR  1904.29(b)(2): What do I need to do to complete the OSHA 301 Incident Report? You must complete an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the OSHA 300 Log.  a) On or about 08/15/24, the employer did not ensure that the Cheney Dynos Inc incident report for an injury that occurred on 07/30/2024 was equivalent to the OSHA 301 incident report form in that it did not have numbers 2, 3, 4, 5, 6, 8, 9, and 10, from the OSHA 301 incident report on their incident report.  b) On or about 10/07/24, the employer did not ensure that the Cheney Dynos Inc incident report for an injury that occurred on 09/07/2024 was equivalent to the OSHA 301 incident report form in that it did not have numbers 2, 3, 4, 5, 6, 8, 9, and 10, from the OSHA 301 incident report on their incident report.

You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.   a) On or about 07/20/2024, the employer did not ensure that one recordable injury where an employee received caustic chemicals in their eye was recorded on the OSHA 300 log until 08/15/2024.  b) On or about 07/30/2024, the employer did not ensure that one recordable injury where an employee was hit by a 4" hose above the eye and required stitches was recorded on the OSHA 300 log until 08/15/2024.

29 CFR  1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a) On or about 09/07/2024, the employer did not ensure that a hospitalization for an employee who fell and broke their ribs in a work related incident was reported within 24 hours to OSHA.
