The employer did not ensure that the operating handle for the forklift was intact and that the cover around the battery was repaired.

On or before August 22, 2024, the employer did not ensure that the XN half ton Hoist located in the was removed from service in  that a safety latch on the hook of the XN half ton -underhung hoist was broken. Employees were exposed to crush injuries in that the load could become de-attached from the hook as employees used the hoist to lift 500 pound steel rotary lids in the Cotton Room.  Cheney Dynos was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH Act of 1970 Section 5(a)(1), which was contained in OSHA inspection number1682064, citation number 1 , item number 1, and was affirmed as a final order on 11-16-2023, with respect to a workplace located at 1000 Anderson Street FRANKLIN, OH 45005.
