On or about January 6, 2025, the employer failed to provide effective guarding to protect employees working at or near the RING-011, National Rivet & MFG. Co., Model No. 1000, Serial No. 250, from crush and amputation hazards while the machine operator's hand(s) were within six (6) inches of the riveting zone during normal production operations.
