29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a. Can Press/Shear Department - On or about July 22, 2025, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. A shear machine operator sustained a fingertip amputation of the right ring finger while adjusting the air line on Bliss 1103 Scroll Shear Slitter Die Press #4.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. On or about July 22, 2025, an employee was exposed to machine hazards associated with moving parts while adjusting the Bliss 1103 Scroll Shear Slitter Die Press #4, which resulted in a right ring fingertip amputation. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Production - On or about July 22, 2025, and days prior to, the employer failed to provide effective guarding to protect employees working at or near the Bliss 1103 Scroll Shear Slitter Die Press #4 air line. An employee sustained a right ring fingertip amputation while adjusting the air line. b. Production - On or about July 24, 2025, and days prior to, the employer failed to provide effective guarding to protect employees working at or near the Bliss 1103 Scroll Shear machine #4 flywheel, thereby exposing employees to hazards as those created by rotating parts.
