29 CFR 1910.147(d):29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): a)?On or?about May 29, 2025, in the Can Press Department,?employees were?exposed to?machine hazards?associated with?moving parts?when clearing?a jam from a Stamping Press #25 Machine. The?employer failed?to implement?energy control?application steps?as the?machine was?not shut?down or?turn off?to perform?the servicing?work [per?the 1910.147(d)(2)?requirements]. As?a result,?the remaining?applicable energy?control elements,?involving machine?isolation [(d)(3)],?LOTO device?application [(d)(4)],?dissipation of?residual energy?[(d)(5)((i)], and?verification of?isolation [(d)(6)],?were not?implemented to?protect employees?from machine?servicing hazards.

29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with procedures required by 29 CFR 1910.147(c)(4): a) On or about May 29, 2025, in the Can Press Department, employees were performing a jam removal on the Stamping Press #25 machine and the employer failed to ensure that employees were utilizing group lock out while performing the task, thereby, employees are being exposed to amputation hazards.
