NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level: a - 119 Malibu Drive, Egg Harbor Township NJ: The company worksite Foreman and a worker were exposed to fall hazards, in excess of 19 feet 2 inches to the ground surface below, while working and walking from a residential structure's upper, pitched, partially sheathed roof trusses while wearing full body harnesses whose connected, lanyard's lengths along with the unrated anchor point these lanyards were improperly connected to would not have stopped them from contacting lower levels. Condition observed on or about 03/18/25. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. a) 119 Malibu Drive, Egg Harbor Township NJ: On or about 03/18/25, a Pettibone All-Terrain lift, used to deliver and support a stack of OSB to workers working from the pitched roof trusses of a worksite residential structure, was left unattended with the load engaging means raised and extended, the key left on the lift's operator seat, and the brake not set thereby exposing workers to the hazard of being struck by the vehicle or its supported load.
