Two employees were working in a parked and elevated scissor lift to work on festoon wiring that energized a traversing conveyor system. The conveyor moved and the festoon wiring pushed on the side of the scissor lift. The employees were not able to lower the scissor lift at that point, and when the conveyor moved again, the wiring pushed the scissor lift over. One employee suffered a broken arm and hip and that required surgery and hospitalization. The second employee broke his finger and suffered contusions.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
An employee climbed an aluminum extension ladder inside a grain bin to help remove a stirring machine. As the farmer began to lower the stirring machine from the top, one of the ropes broke, causing the stirring machine to strike the ladder and knock it out from under the employee.
