On [REDACTED] at [REDACTED] a Lost Time Injury occurred in the Warehouse at [REDACTED] A Warehouse Technician (DOH: [REDACTED]) was manually picking from the ground level when they bent at the waist and reached into the bay to retrieve a carton of finished goods As they prepared to stand up they struck the top of their head on the overhead crossbeam of the racking The operator immediately noticed blood on their head and walked to the Unit Managers office First Aid was administered and Axiom was contacted The technician was recommended to seek care at the local emergency department where they were treated for a laceration to the top of the skull ultimately 15 staples were applied for the 4 long laceration They were given OTC medication an order to follow up at OHC on [REDACTED] The Technician was evaluated on [REDACTED] and given the all clear to return to work but began having neck pain that following [REDACTED] They returned to the clinic on [REDACTED] and was given medication and res
