Before the incident
On [REDACTED] at [REDACTED] a Recordable injury occurred in the Warehouse A Warehouse Technician ([REDACTED]: [REDACTED]) was grabbing a pallet from the pallet stack to utilize for picking an order when lifting the pallet with two hands and the pallet was approximately chest high the board he was holding broke free from the pallet and the rest of the wooden pallet fell down During the pallets descent it made contact with the Warehouse Technicians right shin which resulted in a superficial skin tear A Unit Manager responded upon hearing the pallet drop to the ground This is where they noticed the Technicians leg was bleeding profusely He was escorted to the Unit Managers office and First Aid measures were applied Axiom was contacted however unable to stop the bleeding recommendation was made for the Technician to be seen at the local emergency department He was treated for a 6 superficial skin tear resulting in 14 sutures
