Before the incident
On [REDACTED] an operator sustained a contusion to the head The operator (DOH: 31124) was picking up materials on a bottom rack and upon standing struck his head on the metal cross beam While the injury did not produce any injury he did report the incident to his UM Axiom was notified and First Aid measures were recommended On [REDACTED] the operator reported to his UM that he had been experiencing regular headaches since the initial incident Axiom was contacted to reopen the case and they recommended medical evaluation The following day the operator was evaluated by the OccHealth physician and diagnosed with a concussion given medication The operator was released to work full duty with a followup appointment in 2 weeks
