On [REDACTED] at [REDACTED] an operator (DOH:121319) sustained a back strain injury The operator was in process of picking pails from the ground level in location JD0241 Upon picking the 5th pail ([REDACTED] approximate weight is 56 lbs) from the pallet the operator felt a pinch in the lower back area They reported the pain immediately to the Unit Manager and Axiom was notified First Aid recommendations were given and the site made accommodations Over the following 10 days daily followup was had between the operator Unit Manager EHS and Axiom to ensure positive progress Additionally the operator met with the onsite Chiropractor to evaluate the affected area (First Aid) However on [REDACTED] the operator had not shown significant improvement and requested a medical evaluation at the OccHealth Clinic During this evaluation they were diagnosed with a lumbar strain given prescription medication and restrictions in which the site is unable to accommodate This is a Lost Time injury
