Before the incident
On [REDACTED] at [REDACTED] the site experienced a Level 1 injury requiring emergency service response The operator (DOH: 01162024) was operating an Order Picker turning into the aisle when his printer dislodged from the resting location The operator attempted to catch stop the printer and in doing so caused him to strike the upright of the stationary racking The force of striking the rack caused the employee to lose his balance and fall from the operators platform of the Order Picker simultaneously his right foot became pinned underneath the equipment Another operator overheard the incident and responded (via radio communication) the medical emergency code The employees right foot was pinned under the equipment such that a HiLift equipment was utilized to release the operator Given the severity of the injury emergency services were contacted and the operator was transported to the local hospital for treatment The operator was diagnosed with multiple fractures and due to the open lacer
