On [REDACTED] the site experienced a Level 1 injury when an operator sustained a lower lumbar sprain The operator (DOH: 12423) was performing picking functions from the platform on the Order Picker to 2nd level rack in Aisle [REDACTED] The operator raised the Order Picker to the 2nd level then proceeded to reach into the back of the location to retrieve the pail which was double stacked on the pallet picked it up enough to turn it on its side and roll forward As they were lifting the pail to place on the pallet they were building the operator became distracted by the scan gun falling from the holster turned toward the distraction and felt a pain in the lower back The incident was reported to the UM and Axiom was contacted During a followup call with Axiom the operator stated their back pain was not improving and requested to been seen at the OccHealth clinic The operator was evaluated and diagnosed with a lumbar strain sprain given medication and work restrictions (which the site can a
