Before the incident
Unloading a trailer
What happened
On [REDACTED] @ [REDACTED] entered the trailer without first checking to see if the trailer was properly docked and if he had proper clearance. As he entered the trailer it lowered due to the driver not being docked with his air bags filled. This would have raised the trailer to meet the plate and give the operator the proper clearance. As the EE exited the trailer with a pallet of screens the front bottom of the electric pallet jack base struck the loading dock plate due to height clearance. As the pallet jack swerved EE simultaneously lost control and pinched his right ankle between the left side base of the jack and the opening of the door.
