Seaming glass

Around [REDACTED] the injured employee was seaming glass with the other coworkers. The work order was called for 4.7 glass in thickness. [REDACTED] was performing his task on the seamer [REDACTED] 8. His duties were to receive glass from the cutting table operator then proceed to seam two sides of the glass and pass it to the next seamer 29-Lewis [REDACTED] to seam the other two sides who was working on the [REDACTED] 9. At [REDACTED] [REDACTED] finished the two sides on a glass and passed it to [REDACTED]. At this point [REDACTED] was facing and talking with [REDACTED] and not paying attention to the other piece of glass that the table operator had already sent to him. This short time of distraction caused the piece of glass to run across the whole ball-table without the employee s attention and fall down behind where [REDACTED] was located causing the piece of glass to break and cause a laceration on [REDACTED] s calf.
