What happened

On [REDACTED] at approximately [REDACTED] hrs. Security Supervisor [REDACTED] was dispatched to the medic 's office for an employee injury ([REDACTED] II [REDACTED]). Upon arrival and under surveillance [REDACTED] made contact with [REDACTED] and medic [REDACTED]. [REDACTED] was advised by [REDACTED] that she was cutting lemons in the Fresh [REDACTED] dessert bar area and cut her left-hand thumb causing her thumb to bleed. [REDACTED] added pressure to control the bleeding. After getting the bleeding under control [REDACTED] wrapped her thumb in gauze. [REDACTED] issued [REDACTED] paperwork for Working Well Facility and [REDACTED] left property with no further incident to seek further treatment on her own. [REDACTED] was unable to fill out a voluntary statement Worker 's Comp form or sign a refusal waiver at this time due to the severity of the injury. AIR Form and Accident Scene Checklist have been completed. [REDACTED] will complete the remaining medical forms once she returns to work
