On [REDACTED] at approximately [REDACTED] Security Supervisor [REDACTED] responded to the Medic’s Office for an Employee Injury Slot Technician [REDACTED]. [REDACTED] stated that while working on a Slot Machine in the Slot Tech Storage area he cut his hand on the metal BVA Box Door. Medic [REDACTED] examined and bandaged a small cut on [REDACTED]’s left index finger. Slot Technician Supervisor [REDACTED] was notified of the incident. [REDACTED] signed a refusal of transport further care completed a Voluntary Statement and completed an [REDACTED]. An AIR Form Accident Scene Checklist and Follow Up Information Checklist were completed regarding the incident. Surveillance was notified and had coverage throughout the incident. Surveillance stated that upon review [REDACTED] was working on a Slot Machine in the Slot Tech Storage area and that at [REDACTED] [REDACTED] was observed checking his finger where the injury had occurred.
