What happened

On [REDACTED] at approximately [REDACTED] Security Supervisor [REDACTED] was dispatched to the Medic’s Office for an injured employee identified as [REDACTED] Houseperson [REDACTED]. [REDACTED] stated that while he was walking on kegs in the beer line room of liquor storage to release a flow line valve one the kegs fell away causing him to fall and hit himself on the shelves but not all the way to the ground. Medic S. [REDACTED] assessed [REDACTED] 's condition. [REDACTED] stated that his neck was stiff and that his left torso was sore. After assessment [REDACTED] refused any further treatment and signed a medical refusal of treatment and transport to the hospital by ambulance. [REDACTED] then completed all medical paperwork and issued [REDACTED] a treatment authorization form. [REDACTED] then returned to work for the remainder of his shift without further incident after completing all medical paperwork.
