On [REDACTED] at approximately [REDACTED] Security Supervisor [REDACTED] was approached by Rock Shop Clerk [REDACTED] to report an injury that occurred while working on [REDACTED]. [REDACTED] stated that she did not report the incident due to being by herself at the time and would report it the next day. [REDACTED] also stated that while mopping in the Rock Shop she felt a sharp pain in her back and it progressively got worse. Rock Shop Manager [REDACTED] was notified. [REDACTED] completed a voluntary statement of the incident Worker’s Compensation Worksheet and was issued Working Well information and went home to seek further medical treatment. An AIR Form Follow Up Information Checklist and Accident Scene Checklist were completed regarding the incident. Surveillance review of the footage shows [REDACTED] grabbing her back and leaning over after mopping at approximately 2327hrs and again at [REDACTED] and could not determine the cause of the injury.
