On [REDACTED] at approximately [REDACTED] Security Supervisor [REDACTED] was dispatched to the medic’s office for an injured team member. Upon arrival Surveillance was contacted for coverage. [REDACTED] then spoke to [REDACTED] Cook [REDACTED] who stated that he cut his left pinky finger while cutting greens in the Council [REDACTED] kitchen. Medic [REDACTED] was able to clean the wound and bandaged [REDACTED] finger. [REDACTED] signed a medical refusal and all other medical paperwork. [REDACTED] ended his shift and went to seek further medical attention at working well. Surveillance informed [REDACTED] that at approximately [REDACTED] they observed [REDACTED] reacting to something. [REDACTED] later [REDACTED] was observed leaving the kitchen and going to the medic’s office.
