On [REDACTED] at approximately 0955hrs Security Shift Supervisor [REDACTED] and Shift Manager [REDACTED] were dispatched by surveillance to the [REDACTED]’s Office in regards to an injured employee. Upon arrival [REDACTED] met with [REDACTED] [REDACTED] who was speaking to team member [REDACTED]. Mr. [REDACTED] stated that he pinched his left pinky finger while lowering a dock plate for a vendor (see statement). [REDACTED] [REDACTED] applied a bandage to Mr. [REDACTED]’s left hand and a transport refusal was signed. [REDACTED] returned to work the rest of his shift.
