TM was walking through the employee lot.

On [REDACTED] at [REDACTED] 0350hrs Security Supervisor [REDACTED] was informed of an employee injury that took place at [REDACTED] involving Table Games Supervisor [REDACTED]. [REDACTED] stated that she slipped and fell while walking into work in the employee parking lot. [REDACTED] stated that she reported that she fell to her Manager [REDACTED] at [REDACTED] [REDACTED] when she arrived at the building. [REDACTED] was advised by Medic S. [REDACTED] that he could not do much for her but should she feel any real serious pain she should seek medical treatment. [REDACTED] stated that at the time of her falling she felt a bit of pain in her left wrist and knee. [REDACTED] then completed all the medical paperwork and supplied [REDACTED] with a Working Well Authorization form so that should she need to seek medical care she could. [REDACTED] signed and medical refusal form supplied a statement and then remained on property to complete her shift without further incident.
