What happened
On [REDACTED] at approximately [REDACTED] hrs. Security Supervisor [REDACTED] was dispatched to the medic off for an injured employee ([REDACTED] 21567). Under surveillance coverage [REDACTED] and medic [REDACTED] was advised by [REDACTED] that he was working in the [REDACTED] dish area. [REDACTED] stated that he was washing dishes in the dish area as he was putting away some steel bread plates one of his co-workers were cleaning some wood board over his head and dropped one of the wood boards onto the steel bread plates crushing his right hand that was underneath the steel plates. [REDACTED] complained of pain to his right hand. [REDACTED] issued [REDACTED] an ice pack and placed his arm in a sling. [REDACTED] asked [REDACTED] if he wanted to be transported to the hospital by ambulance. [REDACTED] declined and advised [REDACTED] that one of his family members will be here to transport him to working well or the hospital. [REDACTED] issued [REDACTED] all medical paperwork and [REDACTED
