What happened

On [REDACTED] at approximately [REDACTED]. Security Supervisor [REDACTED] was dispatched to the EDR (employee dining room) for a fallen employee Security Officer [REDACTED]. [REDACTED] was walking through the EDR and slipped and fell to the ground. [REDACTED] was claiming pain in her right shoulder arm elbow and knee. [REDACTED]’s right elbow was swollen. Medic S. [REDACTED] arrived and transported [REDACTED] to Medic’s office for further assessment. [REDACTED] recommended that [REDACTED] get an x-ray to check for and serious injury. [REDACTED] then requested a BLS (basic life support) ambulance be dispatched because [REDACTED] did not feel safe driving herself. BLS was dispatched at [REDACTED] arrived on property at [REDACTED] and left property at [REDACTED] transporting [REDACTED] to Southlake Methodist for treatment.
