On [REDACTED] at approximately [REDACTED] Security Supervisor [REDACTED] was dispatched to the Medic’s Office where Medic [REDACTED] was assessing an injured employee identified as [REDACTED] [REDACTED]. [REDACTED] stated that he was taking trash out to the trash compactor on the dock and while trying to lift a heavy bag of trash out of a cart he felt a pull in his lower back and was dealing with a lot of pain. [REDACTED] stated that he would like an ambulance so that he could get his back checked as soon as possible. At [REDACTED] a BLS ambulance was dispatched at [REDACTED] it arrived on property and at [REDACTED] the BLS left property to transport [REDACTED] to Northlake Methodist for further treatment without further incident. [REDACTED] Supervisor [REDACTED] was informed of this incident. Surveillance investigated this incident and determined that at approximately [REDACTED] [REDACTED] was at the trash compactor struggling to lift the bag of trash from the bin and looked like he
