Walking on the slot floor

On [REDACTED] at approximately 0043hrs Security Supervisor [REDACTED] responded to an employee injury Slot Attendant [REDACTED]. [REDACTED] observed [REDACTED] in the main aisle marble floor between Table Games Pits 5 and 4. [REDACTED] stated that she had slipped on liquid on the marble floor which caused her to fall to the knees. [REDACTED] stated that her left hip and leg were in pain due to the fall. [REDACTED] observed liquid on the marble and notified EVS of the spill. EVS responded and cleaned the spill at which time [REDACTED] escorted [REDACTED] to the Medic’s office. Medic [REDACTED] examined [REDACTED] and provided her with an ice pack. [REDACTED] signed a refusal of transport further care and was then escorted by [REDACTED] to the Security office. [REDACTED] completed a voluntary statement and [REDACTED]. [REDACTED] was then issued Working Well information by [REDACTED] and returned to work without further incident.
