Before the incident
Emptying a .5 cu. yd hopper of scrap caps
What happened
A forktruck driver was emptying a .5 cubic yard hopper of scrap caps into a 40 yard dumpster. The employee unlatched the locking lever with his left hand and inadvertently had his right hand resting on the support frame of the hopper. When the hopper was released it immediately rolled back onto his pinky finger. As a reflex he jerked his hand and he thinks that this caused him to tear the finger off from the tip that was pinched under the hopper. The locking latch was engaged to the point that they had to unlock the hopper to retrieve his glove which contained the tip of his finger
