On [REDACTED] at approximately [REDACTED] a Wayfair associate reported an injury on the left side of his lower back back left leg and left foot while loading a trailer in door [REDACTED]. The associate states that while lifting a box the associates foot slipped and twisted his back the wrong way. The associate reported an initial pain level of 9 when the injury occurred and a pain level of 9 at the time of the report. During the investigation the site found that the incident occurred in door 124 while loading cross dock for [REDACTED]. In the video the associate was seen loading cross dock with another associate. The supervisor approaches the door to get laborshare to go support a different department when the associate reports having back pains and a tingling feeling going down his back left leg and foot. [REDACTED] was the item the associate was loading of a weight of 121.90 pounds. The associate was performing the task for approximately 5 hours and 30 minutes prior to the injury. Th
