Loading boxes into the trailer

The associate started off the night in IXD loading trailer ([REDACTED]) at dock door [REDACTED]. At [REDACTED] the associate notified the receiving supervisor about a box falling on their head and asked to sit down for a few minutes because they weren 't feeling well and were in pain. When the supervisor asked them what happened they answered that they had loaded a box and when they turned around to grab another box the previous box fell and hit them in the head neck and shoulder. They yelled ouch and walked out of the trailer with their hands on their head while letting their coworker know what happened. They said the box was rectangular and weighed about 55 lbs. They said the pain level at the time was a 9 out of 10 and that they felt fatigued after the incident.
