On [REDACTED] a TIG work crew was performing maintenance activities in one of Citgos process units when a flash fire occurred resulting in injuries to four employees. Plant ERT responded and all four employees were transported to a local medical facility for evaluation and treatment. The cause of the flash fire is currently under investigation.
