What happened
[REDACTED] [REDACTED] at approximately [REDACTED] [REDACTED] who is in their first two weeks of training was operating [REDACTED] and attempting to reverse into Dock Door 214 on the Receiving side. TM [REDACTED] was accompanied by a trainer on the dock. [REDACTED] was standing and working next to a row of pallets that were staged by Door 214. As [REDACTED] was in the process of reversing [REDACTED] they allegedly noticed that the forks of the walkie were elevated. While noticing this TM allegedly attempted to adjust the fork elevation Limit switch on the walkie which caused it to speed up. [REDACTED] tried to slow down and adjust their turn but in the process the walkie forks clipped a pallet which caused it to make contact with [REDACTED] 's left foot. [REDACTED] got off their walkie immediately and the trainer went up to [REDACTED] to make sure they were okay. The trainer notified a supervisor immediately.
