What happened
On [REDACTED] at approximately [REDACTED] a TM approached a supervisor stating that their back was sore from wrapping the previous shift approximately [REDACTED]. The team member said there was some discomfort in their back stating a pain level of 4.10 when walking and it happened while wrapping a pallet the previous shift. TM wanted to inform the supervisor but did not feel that it was necessary to go to the hospital. TM went upstairs to the shipping office and filled out the incident statement as well as the medical declination form. After finalizing the paperwork the team member decided to use vacation time and leave for the day to prevent any additional pain and discomfort.
