What happened
On [REDACTED] around [REDACTED] TM was dropping off a box Medium size box2-4lb in dock door 76. While TM was putting the box down TM felt pain on the left wrist pain level 5 out of 10. TM came back around [REDACTED] and reported injury. TM signed medical treatment declination form. Around [REDACTED] TM stated pain increased and couldn 't continue picking. Ice bag was given to TM but was ineffective in reducing discomfort from the injury.
