What happened
On [REDACTED] at approximately [REDACTED] team member reported getting struck in the face by a box. Team member states I was picking up an item and when I went to grab it and put it on my pallet I hit another box on my pallet causing the item to hit me in my face. I then went to the bathroom to see if I had a mark on my face after my assignment but then my head started to feel like it was throbbing so I went to look for someone to tell. This happened to me in aisle 49. Team member was taken to the care room and initially treated with ice and acetaminophen. Team member reported experiencing some dizziness and a headache so OSL recommended Medcor be contacted. Medcor recommended team member be seen by outside care within 2 hours. Team member drove themself to the urgent care facility escorted by management in a separate vehicle.
