NONE ACCOMMODATING RESTRICTIONS at CALFIRE Recordable with lost work days RX EE reports while relocating cases of oil he noticed pain in right side to the lower abdomen where the leg connects groin area that radiates down the inner thigh EHS [REDACTED] EE states in phone conversation [REDACTED] at 1049a EE was moving cases of oil to stock the maintenance shed EE felt onset of pain approx 20min after completing task EE seeked initial Urgent Care visit with self insurance on [REDACTED] EE returned to work with ultrasound follow up on [REDACTED] Diagnosed with [REDACTED] contacted WC on [REDACTED] with WC provider EE provides statement attached EE will provide doctors notes Customer is none accommodating to work restrictions this is current CALFIRE policy Email sent to higher authority for notice of non accommodation
