Recordable due to fracture Fibula at [REDACTED] and medical treatment at the clinic first full lost workday is [REDACTED] Initial nurse notes EE reports he was up inspecting a helicopter and slipped falling to the hangar concrete EE says his legs were over the side when he slipped off so feels it was 5 6 feet to the ground He landed on his feet and then fell the rest of the way down He denies striking his head or any other areas except he is having significant discomfort and swelling to his left ankle foot He and his supervisor feel he needs medical attention The first full workday was lost on [REDACTED] [REDACTED] EE submitted a doctors note stating Fracture Fibula at [REDACTED] Duty Work Status unable to work until [REDACTED] date appears incorrect I have contacted the doctor for a revision Pending EHS Mgr statement
