Before the incident
EE repositioned his body and placed his left hand on the face of the tube sheet in approximately the [REDACTED] position and his other hand on the scaffold behind him to push himself up so he could stand and reevaluate removal of the channel head.
What happened
A Turner TSG crew of 4 was tasked with removing the channel head from [REDACTED] in the [REDACTED] unit. The work location was accessed by a scaffold approximately 8 high with two deck elevations. While maneuvering the channel head to the right of the exchanger and away from adjacent piping it rested between scaffolding and the existing piping. The injured employee was seated on the upper scaffold deck while guiding the channel head using a 3ft pry bar. All work was stopped so the crew could determine where the equipment was resting against. EE then repositioned his body and placed his left hand on the face of the tube sheet in approximately the [REDACTED] position and his other hand on the scaffold behind him to push himself up so he could stand and reevaluate removal of the channel head. This action caused movement to scaffold which then allowed the channel head it to become free and rotate towards the tube sheet. During this movement the channel head made contact with the left ring
