What happened
On [REDACTED] at approximately [REDACTED] a Packing Lion employee returned from break and went to the bathroom. While returning to their work area they noticed a rip in their boot and while passing by P&D stopped and asked a coworker to look at the boot. The coworker bent down to assess the shoe’s damage. While standing back up the back of the coworker’s head struck the forehead of the employee causing the employee to become unconscious. The employee fell to the floor and first responders were called. During the evaluation of the employee by first responders the employee went unconscious two more times. The employee was taken to the emergency room where no diagnosis was concluded. However the employee was given a return-to-work date of [REDACTED].
