Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that the winch
used in the extraction area was  equipped with a damaged wire rope,
exposing employees to
potential struck-by hazards if the wire rope were to break.
a) Perry Fiberglass Products, Inc., Avon Lake, Ohio:  On or about February
2, 2010, the
employer did not ensure that the Columbia Winch & Hoist manufactured
winch, Model
WR40000-4A17-L01, located in the extraction area, was operated according
to the manufacture's
instructions in that the wire rope being used on the winch was damaged,
exposing employees
to struck-by hazards if the wire rope were to break.
Among other methods, one feasible and acceptable method to correct the
hazard is to ensure that
the winch and wire rope is maintanined and operated according the Columbia
Winch & Hoist
AC Electric Owner's Manual and to comply with the Wire Rope Users Manual,
forth edition,
published by the Wire Rope Technical Board.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation (
using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition,
documentation demonstrating that abatement is complete must be included
with your
certification.  This documentation may include, but is not limited to,
evidence of the purchase
or repair of the equipment, photographic or video evidence, or written
records.

were not in good repair as they were equipped with damaged wire hoist
ropes and an upper limit
switch that was not functioning.
a) Perry Fiberglass Products, Inc., Avon Lake, Ohio:  On or about February
2, 2010, the
employer did not ensure that the R&M Material Handling Inc. manufactured
4-Ton crane,
located in the west fabrication area was in good repair while being
operated by employees, in
that the wire hoist rope was kinked in several locations, exposing
employees to struck-by and
crushing hazards if the wire rope were to fail from the damage and drop
its load.
b) Perry Fiberglass Products, Inc., Avon Lake, Ohio:  On or about February
2, 2010, the
employer did not ensure that the Number 4/5 R&M Material Handling Inc.
manufactured 4 ton
crane, used for joint lifts in the extraction area, was in good repair
while being operated by
employees, in that the upper limit switch was not functioning, exposing
employees to struck-by
and crushing hazards if the load were raised completely up to the trolley
and freed itself from
the hook.
c) Perry Fiberglass Products, Inc., Avon Lake, Ohio:  On or about February
2, 2010, the
employer did not ensure that the Number 4/5 R&M Material Handling Inc.
manufactured 4 ton
crane, used for joint lifts in the extraction area, was in good repair
while being operated by
employees, in that the wire hoist rope was kinked in several locations,
exposing employees to
struck-by and crushing hazards if the wire rope were to fail from the
damage and drop its load.
d) Perry Fiberglass Products, Inc., Avon Lake, Ohio:  On or about February
2, 2010, the
employer did not ensure that the Number 2/3 R&M Material Handling Inc.
manufactured 4 ton
crane, used for joint lifts in the fiberglass winder area, was in good
repair while being operated
by employees, in that the wire hoist rope was kinked in several locations,
exposing employees
to struck-by and crushing hazards if the wire rope were to fail from the
damage and drop its
load.
Among other methods, one feasible and acceptable method to correct this
hazard is to comply
with the requirements of ASME B30.17 "Overhead Hoist", Section 16-2.4.2
Rope Replacements
and ASME B30.17-2007 "Overhead and Gantry Cranes", Section 17-2.2.1
Operational Tests.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
( using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
