Perry Fiberglass Products, Inc. - Avon Lake, Ohio: On or about February 13, 2014, the employer did not ensure that employees received a medical evaluation prior to providing a respirator for voluntary use.  Employees in Plant 2 utilized a half face tight fitting respirator that can place a burden on an employee's cardiovascular and respiratory systems.    b. Perry Fiberglass Products, Inc. - Avon Lake, Ohio: On or about February 13, 2014, the employer did not ensure that employees received a medical evaluation prior to providing a respirator for voluntary use.  Employees laying fiberglass in Plant 1 utilized a half face tight fitting respirator that can place a burden on an employee's cardiovascular and respiratory systems.

The eyewash station was empty.    b. Perry Fiberglass Products, Inc. - Avon Lake, Ohio: On or about February 13, 2014, the employer did not ensure the self-contained eyewash located on the South end of Plant 1, near the offices, was filled with eyewash solution.  The eyewash station was empty.    c. Perry Fiberglass Products, Inc. - Avon Lake, Ohio: On or about February 13, 2014, the employer did not ensure the self-contained eyewash located on the North end of Plant 2, near the break room area, was filled with eyewash solution.  The eyewash station was less than half full of eye wash solution.    d. Perry Fiberglass Products, Inc. - Avon Lake, Ohio: On or about February 13, 2014, the employer did not ensure the self-contained eyewash located on the South End of Plant 2 was filled with eyewash solution.  The eyewash station was approximately one-third full of eyewash solution.    Perry Fiberglass Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.151(c), which was contained in OSHA inspection number 313781320, citation number 1, item number 4 and was affirmed as a final order on June 30, 2010, with respect to a workplace located at 33660 Pin Oak Pkwy., Avon Lake, Ohio 44012.
