On or about February 21, 2013, at the above addressed jobsite, fall protection was not available for those employees exposed to falls of: (a) Approximately 20 feet (2 stories) while installing roof sheeting on the north side of the structure and, (b) Approximatley 10 feet (1 story) while installing the roof rafters on the west side of the structure.
