29 CFR 1910.119(e)(3)(v): The process hazard analysis did not adequately address facility siting. This violation occurred in the Ozona Gas Plant on or about June 5, 2013 and at times prior thereto where the employer failed to conduct a consequence analysis on the control room using an appropriate methodology such as API RP 752. Pursuant to 29 C.F.R. 1903.19 within (10) calendar days of the abatement date the employer must submit documentation showing it has completed the consequence analysis.
