29 CFR 1910.23(a)(2): Every ladderway floor opening or platform shall be guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening: a) DCP Midstream, LP: On and before 2/11/14, the employer did not ensure the openings for the fixed ladder to the platform on the TEG skid and to the discharge filter was provided with a standard railing, swinging gate. This condition exposed employees to a fall hazard of 10 feet and 20 feet respectively. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) DCP Midstream, LP: On and before 2/11/14, the employer did not ensure the rotating part on the motor of the TEG glycol pump was covered to prevent accidental contact with the rotating part. This condition exposed employees to amputation hazards. Abatement Note: Abatement certification is not required for this item (see enclosed "Certification of Corrective Action Worksheet").
