29 CFR 1904.5(b)(2) Employer did not accurately claim an exception to recording a work-related injury on the relevant OSHA log: (a) DCP Midstream LP, at 3026 4th Avenue, Greeley, CO: On and preceding 4/20/17, the employer did not accurately claim an exception to recording a work-related injury with respect to an injury occurring at the DCP Lucerne 2 plant, when an employee fractured a leg when falling from the top of an eight foot tall bookshelf. The employee was entering into a twelve foot drop-celling plenum in an attempt to access the employee locker room. This injury resulted in days away from work. The employer did not record the work related injury on the OSHA 300 Log.
