The employer does not ensure that before lockout or tagout devices are removed and energy is restored to a machine or equipment that authorized employees inspect the equipment to ensure it is operationally intact.     A.     The violation occurred in the DCP Straight Compressor Station on or about November 20, 2014 and at times prior thereto. Employees were exposed to fire and explosion hazards when the employer failed to ensure employees inspected the Gas Glycol Dehydration equipment after the lockout or tagout devices were removed to ensure the equipment components were operationally intact.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure that before lockout or tagout devices are removed and energy is restored to a machine or equipment that authorized employees inspect the equipment to ensure it is operationally intact.
