29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening.    The employer allowed the barrier safety chain at the top of the ladder to be disconnected from its second connection point in the ladder opening at the time of the inspection, although no employees were on the subject platforms at the time of the onsite inspection.    4) Within the NGL Product area, the Product Separator had a vertical caged ladder   5) Within the NGL Product area, the Product Cooler had a vertical caged ladder   6) Within the NGL Product area, a caged ladder   7) The Hot Oil Heater

29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and addressing at least steps for each operating phase, safe operating limits, safety and health considerations, and safety systems and their functions as outlined by this paragraph.    On or about 22 February 2016, the following written procedures were not produced for review:     1) For the Drain/SLOP Oil System, both the Normal Shutdown and Emergency Shutdown Operating Procedures were missing.  2) For the Gas Treating Facility, the Emergency Shutdown Operating Procedure was missing.  3) For the Instrument Air System, both the Emergency Shutdown and the Normal Startup Operating Procedures were missing.  4) For the Truck Loading/Unloading, the Shutdown, Normal Startup, and Initial/Turnaround Startup Operating Procedures were missing.

On or about 22 February 2016, approximately 18 piping circuits were overdue for thickness testing.

29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use.    On or about 22 February 2016, and at times prior thereto, the employer did not replace pressure safety valves (PSV) that had been identified through 3rd party testing to have either failed or to have discrepancies, exposing personnel working within the process area to a fire and explosion hazard from the release of hydrocarbons or catastrophic rupture of a vessel.    3) PSV-5524 was tested on 2015/06/16. Comment section indicated PSV failed seat leakage and pop test; however, the valve was not replaced.

29 CFR 1904.32(b)(6): The annual summary was not posted between February 1 of the year following the year covered by the records and kept in place until April 30 at the work site.  At the inspection site, the employer did not sign and post the OSHA 300A Summary of Work-Related Injuries and Illnesses by February 1st of the following year within the workplace. 1) The Summary for calendar year 2014 had a signature date of 2/27/2015. 2) The Summary for calendar year 2015 had a signature date of 2/15/2016.
