Clarion Boards, Inc. - On or about October 24, 2013, the company's Exposure Control Plan was not site specific in that it did not identify the following: 1. Person or department responsible for maintaining and providing personal protective equipment. 2. Locations for disposal of contaminated ppe, equipment and clothing. 3. Person or department responsible for offering, and managing records for, Hepatitis B vaccinations. 4. The health care professional responsible for providing the Hepatitis B vaccinations. 5. The person to contact if an exposure incident occurs, including after hours contacts. 6. The health care professional responsible for conducting the confidential post-exposure evaluation and follow-up. 7. The listing of those employees or positions determined to have occupational exposure to blood or other potentially infectious materials.
